Important information: please read

1. Title, First name, Last name, personal email, telephone number, country and trainee status are required for each delegate attending the event. Without this information ISUOG is unable to process membership.
2. Organisers must ensure that each individual delegate has a unique email address, withouth this information ISUOG is unable to process membership.
3. If this form is sent to ISUOG without all the required information completed, it will be returned to the organiser to complete the missing information and membership for the entire group will be suspended until all required information is received.
4. It is the responsibility of the organiser to email this spreadsheet to ISUOG with the information complete. ISUOG will not accept handwritten scanned copies of this form.
5. Information will be used as presented on this form and uploaded into ISUOG's membership database.
